The ultrasound report sits on the table. It says "Grade 2 hepatic steatosis" or "moderately echogenic liver" or simply "fatty liver — Grade 2". The doctor has circled it in red. And now you're home, Googling what Grade 2 fatty liver means for your future — and finding answers that range from "don't worry, just diet" to "you're heading toward cirrhosis". The truth is somewhere specific and knowable. This article explains exactly what each grade means, what the risks are at each level, and what the next investigation should be.

How Fatty Liver Is Graded on Ultrasound

The ultrasound grades of fatty liver are based on echogenicity — how bright the liver appears on the ultrasound image compared to the adjacent kidney and spleen. Fat in liver cells increases the liver's reflectivity to sound waves, making it appear brighter (more "echogenic") on the scan. The grading is a subjective assessment by the radiologist, not a precise measurement — which is why two ultrasound reports on the same liver can sometimes give different grades.

Grade 1 (Mild) Fatty Liver

The liver appears mildly more echogenic (brighter) than the kidney, but the internal vascular markings (the hepatic veins and portal vein branches) are still clearly visible, and the diaphragm is clearly seen. This means fat has accumulated in liver cells but not to the degree that it obscures normal architecture.

What it means clinically: Grade 1 fatty liver is the most reversible stage. Liver function tests are often completely normal. There is no established fibrosis (scarring) at this stage in most patients. A consistent lifestyle change — weight reduction, dietary modification, exercise — can return the liver to normal on ultrasound within 6–12 months.

What to do: No specialist referral urgently required if LFTs are normal. Begin lifestyle modification immediately — this is your widest window of opportunity. Repeat ultrasound in 6–12 months to confirm improvement.

Grade 2 (Moderate) Fatty Liver

The liver is significantly more echogenic than the kidney, and the internal vascular markings are now partially obscured — they are less well-defined than normal. The diaphragm is still visible. This indicates moderate fat infiltration throughout the liver parenchyma.

What it means clinically: Grade 2 is the grade most commonly presented to us at Solo Clinic GI for assessment. Liver function tests are often mildly abnormal — elevated ALT, AST, or GGT. Without intervention, a proportion of Grade 2 NAFLD patients will progress to NASH (inflammation plus fat), which carries fibrosis risk. The metabolic context matters greatly here — a Grade 2 fatty liver in a well-controlled diabetic who exercises regularly is a different situation from Grade 2 in an obese, insulin-resistant, sedentary patient with a family history of liver disease.

What to do: A consultation with a hepatobiliary specialist is appropriate. Blood tests — full LFT, fasting lipid profile, HbA1c, fasting insulin, thyroid function — map the metabolic drivers. A FibroScan (liver stiffness measurement) may be requested to assess whether any fibrosis is already present.

Grade 3 (Severe) Fatty Liver

The liver is markedly echogenic. The vascular markings within the liver are no longer visible, and the diaphragm itself may be poorly seen through the dense, echogenic liver parenchyma. This indicates severe, diffuse fat infiltration throughout the liver.

What it means clinically: Grade 3 fatty liver requires prompt specialist assessment. Liver function tests are almost always abnormal. The risk of NASH and established fibrosis is substantially higher. At this grade, a FibroScan and specialist review are not optional. Reversal of Grade 3 fatty liver takes longer and is less complete — but is still worthwhile and achievable with committed lifestyle change and metabolic treatment.

What to do: Hepatologist or hepatobiliary surgeon assessment. FibroScan to stage fibrosis. Comprehensive metabolic workup. A structured, medically supervised weight loss programme rather than self-managed dietary change.

⚠ When Fatty Liver Grading Is Not Reliable

Ultrasound grading of fatty liver is subjective and can be affected by the quality of the machine, the skill of the sonographer, the patient's body habitus, and even the fasting state at the time of scan. If your ultrasound grade seems inconsistent with your clinical picture — for example, Grade 3 fatty liver with normal LFTs and a thin physique — a repeat scan or alternative investigation (FibroScan or MRI-based fat quantification) may give more reliable information.

What LFTs Add to the Ultrasound Grade

Liver function tests (LFTs) — specifically ALT (alanine aminotransferase) and AST (aspartate aminotransferase) — measure liver cell injury. Elevated ALT in the context of fatty liver suggests active hepatocyte damage, pointing toward NASH rather than simple steatosis. The degree of ALT elevation does not directly correlate with ultrasound grade, however — it is entirely possible to have Grade 3 fatty liver with only mildly elevated ALT, or Grade 2 fatty liver with more substantially elevated ALT due to active inflammation.

GGT (gamma-glutamyltransferase) is often elevated in both NAFLD and alcohol-related liver disease — it is not specific but is sensitive, making it a useful screening marker. ALP (alkaline phosphatase) elevation suggests bile duct involvement and should prompt further investigation.

FibroScan: The Non-Invasive Fibrosis Test

FibroScan (transient elastography) uses low-frequency ultrasound waves to measure liver stiffness — a surrogate marker for fibrosis. A stiffer liver means more scar tissue. It is painless, takes 5 minutes, and provides a fibrosis score (F0–F4) and a steatosis score (S0–S3) on the CAP parameter.
